Your first visit to Donato Chiropractic is an important one. During the visit, we work with you and obtain important background information and medical history. You will also get an assessment of your condition.
You may schedule an appointment via email or on the phone by calling or texting (518)-538-8200. As a new patient, your initial consultation appointment will take around 60 minutes. Follow-up appointments typically take 30 minutes. Please call our office with 24 hours advance notice if you need to cancel or reschedule your appointment. We look forward to seeing you!
What to bring
Prior to your first visit, you may print and fill out our new patient intake form by clicking on the link below. Please bring it to your appointment to save some time! If you are unable to complete this form in advance of your appointment, we appreciate you arriving 10-15 minutes prior to your scheduled appointment time in order to complete it in our office.
During your first visit, Dr. Donato will have a discussion with you about your health concerns and how you can improve your wellness. He will perform a focused physical examination, including a variety of assessments, to appropriately diagnose your condition and determine whether or not you are a good candidate for chiropractic care. If the doctor determines that chiropractic care is a good fit for you, a treatment plan will be created to best fit your condition.
What to wear
Active Release Techniques® (ART) work best when patients wear loose clothing, or where practical – garments that allow for contact with the skin near the affected area. Many patients choose to change into shorts, short-sleeved shirts, or athletic apparel prior to treatment for the best results.
